Add option to provide comment to categories

Based on the feedback received, it would be good to include option to provide comment to each response. The comments should be used to provide context and additional related information, such as evidence for future use.

The comments should be included in the report and should be included in the state. (we need to investigate if the state can be easily stored as the URLs have limit, and it probably would not be possible to share using the URL...)

This feature is a probably good candidate for improvement with the shared state.
